The Elder Outreach Program serves Taunton residents, sixty years of age and older. The Elder Outreach Program provides in-home case management services in the languages of English and Portuguese.
The Elder Outreach Program is funded in part by the City of Taunton, a Formula Grant from the Executive Office of Elder Affairs and Community Development Block Grants from the Executive Office of Economic and Community Development.
The City of Taunton Department of Human Services / Taunton Council on Aging provides elders, sixty years of age and older who reside in the City of Taunton, with the Elder Outreach Program. The Elder Outreach Program is a team of professionals who provide bilingual (Portuguese and English) in-home case management and support services, friendly visiting, in-home non-invasive nursing services, application completion, advocacy and information and referral.
The staff at Elder Outreach is trained to provide elder citizens with understanding persons to help them deal with the problems they may encounter in their later lives. One goal of the team is to aid elders to cope with the loneliness they may feel and to provide these individuals with companionship and with a network of persons and services who care.
The Elder Outreach team urges elder persons who are feeling lonely and / or having difficulty dealing with problems to contact them at 508-821-1425. We also encourage individuals of any age or agency staff to call if they should know of any elder who could benefit from the services of Elder Outreach.
